Employee Benefits

Medical Insurance: United Healthcare PPO

We offer a PPO medical plan for all eligible employees and dependents. You have access to a network of physicians, specialists and facilities. Benefits are available for office visits and hospital care, as well as inpatient and outpatient surgery.

401(k): Nationwide

All full and part-time employees are eligible for this benefit on their first day of employment. The State National Companies 401(k) Profit Sharing Plan allows employees to contribute a flat dollar or percentage of income pre-tax, up to the maximum allowable by the IRS. The company matches 50% per dollar contributed up to the first 6% with an additional matching option based on profitability. Full vesting occurs after 6 years of service, with partial vesting before 6 years.

Dental Insurance: MetLife

As a dual-option PPO, it provides coverage for diagnostic and preventive care at 100%, restorative and other basic services at 80% or 50% after deductible and major restorative at 50% after deductible. Both plans also provide orthodontia coverage for dependent children up to age 19.

Vision Insurance: Spectera

As a PPO plan, it provides coverage for participating providers at the co-pay of $10 for eye exams and $25 for materials (frames and/or spectacle lenses or contact lenses). Reimbursements for out-of-network providers are also available.

Life Insurance and Accidental Death & Dismemberment:

Company paid benefit that pays one times your annual salary, capped at $50,000. Plan features includes Portability/Conversion privilege.

Voluntary Life Insurance:

Option to purchase additional life insurance coverage for yourself, spouse and/or dependents in various increments at discounted group rates.

Long Term Disability Insurance:

Company paid benefit that pays 60% of salary to a maximum benefit of $1,000 per week. Plan features include rehabilitation and full maternity benefits and no exclusion for pre-existing conditions.

Short Term Disability Insurance:

Optional benefit that pays 70% of salary to a maximum benefit of $1,000 per week. Plan features include rehabilitation and full maternity benefits and no exclusion for pre-existing conditions.
